• To give patients access to the insulin calculator, request Accu-Chek Bolus Advisor Protocol sheets from your Accu-Chek rep. Each sheet has a one-time-use HCP code for you to authorize patient access to this feature. If you aren’t currently working with a rep, contact Accu-Chek Customer Care at 1-800-858-8072.

    Protocol sheets also enable you to record specific information about the patient’s therapeutic plan, which you’ll need to set up the patient’s app during their appointment.

    The Accu-Chek Bolus Advisor, as a component of the Accu-Chek Connect diabetes management app, is indicated for the management of diabetes by calculating an insulin dose or carbohydrate intake based on user-entered data. Before use by carbohydrate counting patients, a physician or healthcare professional must activate the bolus calculator and provide the patient-specific target blood glucose, insulin-to-carbohydrate ratio, and insulin sensitivity parameters to be programmed into the software. Before use by assigned meal bolus patients (age 22 and older), a physician or healthcare professional must activate the bolus calculator and provide patient-specific target blood glucose, assigned meal insulin doses, and insulin sensitivity parameters to be programmed into the software.

     

    WARNING: Do not use bolus advice with neutral protamine Hagedorn (NPH) or any other intermediate-acting insulin. Do not use bolus advice if basal insulin dose is unstable. Long-acting insulin should not be used as a meal bolus or as a correction bolus.

  • To activate the insulin calculator, open the app on the patient’s phone and tap Advisor.

  • Select Assigned Meal Dose, then tap Next.

  • Enter the HCP code from the protocol sheet into the field and tap Start.

  • Enter the Assigned Insulin Dose for each meal and snack, Insulin Sensitivity, Target Range and Maximum Bolus settings you recorded on the protocol sheet.

    Touch the toggle button next to I have confirmed my settings and tap Finish.

  • Read about logging Active Insulin then tap Next, then tap Done.
